It’s Your Right to “Die Well”: Advance Care Planning


Course Description:

This course will give individuals the information and resources they need to be prepared at the end of life. This course will assist individuals with documentation, resources and knowledge of what should be done in the event they become seriously ill or lose decision-making capacity.


Course modules will cover:

  1. What advance care planning is and why it’s necessary.
  2. What documents are needed to be prepared.
  3. What additional details should be considered.
  4. Informing love-ones of your wishes.
  5. Where to find reliable community-based guidance and resource.

End-of-life decisions should not be made at the end of life, so you do not have to be an elder to receive Elder Care Doula services. As an Elder Care Doula, I can help you plan for and relieve the financial and emotional stressors that so many individuals experience during the end of life. I will share my knowledge and resources while providing guidance in several areas. Let me help guide you to the peace of mind you and your 

love-ones deserves.


Services include:

  • Legal documentation preparation and review (Healthcare Advance Directive, Power of Attorney, Living Will, Physician Orders for Life-Sustaining Treatment-POLST)
  • Funeral planning
  • Identifying community resource
  • Long-term care planning

 Your loved-one’s end-of-life should be painless and free from suffering.  Dying peacefully requires rest, you cannot appropriately rest when suffering. The dying process should be a holistic experience, leaving the person we love feeling mentally, physically, emotionally, socially, and spiritually whole. An End-of-Life Doula can provide much needed support.


As an End-of-Life Doula I will provide care support and represent, both the patient and the family throughout the dying process. As a Doula, I will provide support and information on communicating with medical staff and relaying wishes and desires to others. As an End-of-Life Doula, I will defend the needs, wishes, and rights, of the patient and family during all stages of the dying process.


As a Doula, I will take the time to understand the patient’s needs, the patient’s wishes, the family’s wishes, and the family dynamic. I can provide any necessary extra support and relief to all involved. I will intimately tailor services to provide the most effective and comprehensive care for the patient and their loved ones.


Services include but are not limited to:

  • Patient Advocacy

  • Understanding of physical changes during the dying process
  • Coping Techniques
  • End-of-Life Planning resources and support (i.e.: funeral plans, legal doc Bereavement Support)
